- 1. Annual Practicing Certificate (APC) – “Jadual Keempat” or else Retention of Name (RON)
- 2. Lembaga Jururawat Malaysia Certificate (LJM) – “Jadual Kelima”
- 3. Diploma/Degree & Post Basic Certificate (if applicable)
- 4. Diploma/Degree & Post Basic Transcript (if applicable)
- 5. SPM Certificate (Original and English)
- 6. Passport (If have, or else NRIC)
- 6. Marriage Certificate (Original and English – If Applicable)
- 7. SNB Application Form (Sample)
- 8. EP_SP Application Form (Start filling up from Page 4, Sample)
- 9. Declaration of Past Employment Form (if any service gap more than 1 year, need service gap letter), and [If applicable – Certificate of Employment (COE by current or ex-employer)]
- 10. Passport
- 11. Photo (with white background, size: 400 x 514px) – You can WhatsApp Kai your selfie photo (As a passport photo) against the plain colour wall, Kai will edit it white background and resize it for you
- 12. LOA Offer Letter
- 13. COVID-19 Vaccination Certificate (Video guide how to download)
Requirement for SNB Application:
- All documents must be in English, or else need to be translated by a Notary Public/ High Court/ Lawyer.
- Submit colour-scanned copies (preferred) of documents.
- For any documents submitted: – Direct colour-scanned documents of at least 300 dpi are preferred (High Quality). Grayscale copies (also minimum 300 dpi) are also acceptable. – Copies of documents submitted must be clear and all pages of documents must be seen and not cropped.
- The CTC must be initiated on every page of the document and include the full name and signature of the authorised officer and the date of certification. The CTC stamp must be in English.
